VSCode memo extension

Use this with your Ka open by side, for now.

Daily notes
- Daily notes are of the form YYYY-MM-DD.md. The VSCode memo extension has a command to create them.
Multiple notebooks
- In the UI, report these otherwise conflicting note files anyway, in case they were created by accident using the VSCode memo extension (its Ctrl+Click)
Renaming filenames and titles together
- Files should not be renamed on its own if you use Filenames as title. When renaming the filename you should also rename all the references. The VSCode memo extension does this automatically.
